To request a transcript, please print and fill out the pdf form below and return it to the Registrar's Office via fax or mail. The first transcript is free, however for any transcript after that, there will be a $5 charge.
For each separate request, we will need a separate form. Please fill in the address, email address or fax number you want the transcript sent to, legibly.
*After you fax it please call the Union College Business Office to pay the fee over the phone with a debit/credit card. they can be contacted by calling 606.546.1203.
Please include payment when mailing your request.
